The services we offer to foster health for our patients

  • Care Teams

    Your care team will work with you and with each other to improve your health. Each team includes a doctor, nurse practitioners, nurses, medical assistants, and a therapist.

    At a Glance:

    General Medical Care for All Ages

    • Treating illness and injury
    • Preventing illness (including screenings for cholesterol, mammograms, etc.)
    • Chronic care (high blood pressure, diabetes, obesity, depression, etc.)
    • Screening for drug or alcohol problems, depression, and trauma
  • Pregnancy Care and New Baby Care

    Your care team will help you to plan for your family, to give birth, and to care for your newborn child.

    At a Glance:

    Pregnancy Care:

    • Groups for pregnant women and their families (Centering Pregnancy and Centering Parenting)
    • Individual pregnancy care
    • Childbirth Doula –a trained professional who gives support to the mother before, during and just after birth
    • Ultrasounds
    • Colposcopy

    New Baby Care:

    • Regular check-ups for mother and baby
    • Breastfeeding support
  • Healthy Living

    Your care team will help you set achievable goals for improving your health.

    At a Glance:
    • Dieticians to help you explore and address nutrition concerns
    • Healthy Living Groups to share information and support for making healthy changes
  • Counseling

    Your care team includes a behavioral health therapist who can help you deal with stress and anxiety, with family issues, and with attitudes that contribute to a healthy life.

    At a Glance:

    Counseling for our patients for the treatment of:

    • Feelings of fear, depression or confusion
    • Drug or alcohol addiction and recovery
    • Individual, family and couples therapy
  • Doctor on Call

    Speak to a doctor during times when Maple City Health Care Center is closed.

    At a Glance:

    There is always a doctor on-call for our patients, even when the office is closed. Please call the doctor on-call any time you need medical advice or medical care after hours (evenings, holidays, Saturdays, Sundays).

    Unless it is a life-threatening emergency, please call before you go to the emergency room. The doctor on-call can help you decide what to do.

  • Dental Care

    Your dental care team will help you with routine cleanings, fillings and extractions, and other dental services.

  • Access to Health Coverage

    Vista Navigators - Araceli (L) and Adela (R)

    Our health insurance navigators will help you find, apply for, and maintain your health insurance.

    At a Glance:

    We help people in our community as well as people who are our patients with:

    • Applications for Medicaid or other kinds of health insurance
    • Medicaid questions & renewals
    • Using and understanding your insurance coverage
  • Social Services

    We help our patients find solutions to problems such as housing, food, employment, immigration, and transportation.

    At a Glance:

    We help our patients with:

    • Applications for food stamps (SNAP), disability, & financial assistance from Goshen Hospital
    • Housing, immigration, and financial concerns
    • Parenting support
    • Support with accessing other community resources
